난치성질환 번역(한국어 원본)본 연구는 희귀·난치성질환(Rare Dieseases) 자조집단(self-help group)에 대한 국내 연구가 거의 전무한 실정에서 희귀·난치성질환 자조집단이 환자와 가족의 임파워먼트에 미치는 영향을 연구하여 희귀·난치성질환 자조집단 활성화 지지의 근거를 마련하고자 하는 목적으로 수행되었다.
이러한 목적을 달성하기 위해서 선행연구와 문헌조사, 설문조사, 질적 사례연구를 병행하여 국내 희귀·난치성질환 자조집단의 현황을 파악하고 환자 및 가족의 임파워먼트에 미치는 영향을 분석해보고자 하였다.
설문조사는 국내 희귀·난치성질환 환자와 가족을 대상으로 진행하였으며, 총 229부를 최종 자료 분석에 사용하였다. 설문조사의 내용은 조사대상자에 대한 일반적 사항과 의료적 사항, 자조집단 참여여부와 만족도 분석, 개인적 차원의 임파워먼트 개념인 자기옹호력(자기효능감, 자아존중감)과 사회적 차원의 임파워먼트에 대한 사항으로 구성하였다. 또한 질적 사례연구는 국내 희귀·난치성질환 자조집단의 현황을 파악하기 위해 희귀·난치성질환 자조집단의 연합체인 (사)한국희귀·난치성질환연합회 실무책임자 면접을 통해 진행하였다.
본 연구를 통해 입증된 주요 결과를 요약하면 다음과 같다. 첫째,자조집단의 질환극복 도움정도에 따라 희귀·난치성질환 환자 및 가족의 임파워먼트 정도에 차이가 있음을 확인할 수 있었다. 질환극복 도움정도가 높을수록 개인적 관점에서의 임파워먼트인 자기효능감과 자아존중감, 사회적 관점에서의 임파워먼트인 심리적 태도와 집단정체성 및 이 둘을 합한 임파워먼트 모두의 정도가 높은 것으로 나타났다.
둘째, 희귀·난치성질환 자조집단 참여도에 따라 환자 및 가족의 임파워먼트에 차이가 나타났다. 자조집단 참여도가 높은 환자 및 가족집단의 자기효능감, 자아존중감, 임파워먼트 심리적태도, 임파워먼트 집단정체성, 임파워먼트 정도가 참여도가 낮은 집단에 비해 높은 수치를 보였다. 상기 두 결과를 통해 희귀·난치성질환 자조집단이 환자 및 가족의 임파워먼트를 향상시키는 역할을 한다는 것을 검증하는 것이다. 자조집단은 환자와 가족의 자기효능감, 자아존중감을 향상시켜 당면한 문제를 해결하고자 하는 의지와 자신감, 대인관계의 적극성이 높아지며, 문제를 실질적으로 해결하고 공동의 목표를 이루기 위한 행동 능력인 사회적 관점의 임파워먼트 향상에 기여하는 것이다.
셋째, 본 연구에서 주목할 만한 점은 치료법의 존재여부에 따른 희귀·난치성질환 환자 및 가족의 임파워먼트의 차이는 없는 것으로 나타났다는 것이다. 이는 희귀·난치성질환 환자 및 가족에게 있어 직접치료에 대한 지원 뿐만 아니라 정서적·심리사회적 지지가 중요함을 나타내는 것이다.
본 연구는 희귀·난치성질환 자조집단이 환자 및 가족의 임파워먼트에 미치는 영향을 연구한 유일한 논문이며, 희귀·난치성질환 자조집단의 활성화를 위한 지원 필요성의 근거를 마련했다는 의의가 있다. 희귀·난치성질환 환자 및 가족의 삶의 질을 향상시키기 위해서는 직접적인 의료비 지원과 병행하여 희귀·난치성질환 자조집단을 활성화해야 한다. 이를 위해서는 자조집단에의 참여도를 높이고 질환 극복에 도움이 되는 프로그램의 지속적인 개발 및 시행이 중요하며, 그 방법은 자조집단별 개별지원이 적합하다. 또한 희귀·난치성질환 자조집단은 비전문가 집단이므로 희귀·난치성질환 관련 사회복지전문가 육성하여 자조집단 운영 지원 및 자문의 역할을 수행하도록 해야 한다. |
난치성질환 번역(영어 번역본)With very limited cases of studies on the self-help group of rare diseases in the past in Korea, this study was performed to establish a foundation for encouraging self-help groups for rare disease patients by studying about the effects of self-help group of rare disease to the patients and their families.
In order to achieve this, the previous studies and related documentations were reviewed; surveys and quality case studies were also reviewed to understand the current status of Korea’s self-help group of rare diseases and their effects to the patients and families’ empowerment.
Surveys were run for Korea’s rare disease patients and their families and the total of 229 cases were used for the final analysis. Survey questionnaire consisted of subjects’ personal details, medical details, analysis on self-help group participation and the level of satisfaction, self-protection (self-effectiveness & self respect) which are an empowerment in personal sense and empowerment in social sense respectively. As for quality case studies, in order to identify the current status of the self-help group of rare diseases, the Korea Rare Diseases Society staffs were interviewed.
The verified results of this study are as the following. Firstly, it was verified that there are differences in the level of empowerment for the rare disease patients and their families according to the level of help given in terms of overcoming the illnesses to the self-help groups. Higher the level of help given, it was shown that the empowerment in personal sense, namely the self-effectiveness and self-respect and the empowerment in social sense were all high.
Secondly, the result showed that there are differences in patients and their families’ empowerments depending on the participation of the rare diseases’ self-help group. That is those patients and families with high level of self-help group participation had relatively higher levels of self-effectiveness, self-respect, psychological attitude, better self identification and the actual level of empowerment compared to those with low level of participation. The above two results validates the fact that rare diseases’ self-help group enhances the empowerment of the patients and their families. The self-help group improves the patients’ and their families’ sense of self-effectiveness and self-respect which in turn enhances the willingness, confidence and proactive personal relationships to resolve the problems at hand. It actually contributes to empowerment enhancement from the social perspective which is an ability to resolve problems realistically and realize common goals.
Thirdly, one thing to note in this study is the fact that empowerment of the rare disease patients and families is not dependent on the availability of suitable treatment for the disease. This implies that psychological and social supports are also important in addition to direct treatments for the patients.
This study is an only study that deals with how rare diseases’ self-group affects the patients’ and families’ empowerment. It was a valuable study in that it provides a foundation by which to vitalize the rare diseases’ self-help group program. In order to improve the lives of the rare diseases patients and their families, self-help group program must be vitalized in addition to direct medical funding. And to achieve this, it is very important to increase the level of participation in the self-help group activities and continual development of programs that may aid in overcoming such diseases should be carried out. Independent and exclusive support for each of the group would be very appropriate. Also, as rare diseases’ self-help group is not yet a professional group, it is important to nurture social workers who are enabled in such program and enable them to operate, support and advice such activities. |
