(Vendors, contractors and subcontractors) Prior to starting work with Black Box Network Services all suppliers shall provide a certificate of insurance to the following: “Suppliers” shall be covered during the period of the work by policies of insurance as set out below. The certificates of insurance shall provide for a thirty (30) day notice to Company, prior to any effective cancellation date and contain the following coverage and limits: 1 Workers’ Compensation insurance—statutory coverage in the state the work is performed.
• We will need an “all states endorsement”, list of qualified states, and/or copies of all monopolistic certificates of insurance to show coverage in any state where you do work for us.
• Waiver of Subrogation Applies in Favor of Black Box Network Services.
1 Employer’s Liability of $1,000,000 each accident; $1,000,000 disease each employee and
$1,000,000 disease policy limit.
1 Property Coverage (if needed)
1 Automobile Liability including: Hired automobile and non-ownership liability, for bodily injury and property damage at a $1,000,000 combined single limit for each occurrence.
1 Comprehensive General Liability, including: Products and Completed Operations, Contractual,
Broad form contract and broad form property damage with minimum limits of liability of $1,000,000 each occurrence; $2,000,000 general aggregate.
Automobile Liability and Commercial General Liability the following shall apply:
• Additional Insured shall apply in favor of Black Box Network Services and the Owner, their respective directors, officers, employees, subsidiaries and affiliated companies.
• Waiver of Subrogation Applies in Favor of Black Box Network Services and the Owner, their respective directors, officers, employees, subsidiaries and affiliated companies.
• Subcontractors insurance shall be primary and non-contributory to any insurance available to Black Box Network Services and the Owner, their respective directors, officers, employees, subsidiaries and affiliated companies.
1 Excess Liability Policy (Umbrella) of at least $1,000,000 for each occurrence and in the aggregate.
1 Professional Liability – (as required) Any covered loss is to be claimed by the insured as their interests may appear. “Supplier” shall be prohibited from entering upon the project site unless the subcontractor is insured in accordance with the above requirements. Shall the “Supplier”, fail to maintain required insurance which will protect its interests and Black Box Network Services shall charge the “Supplier” the cost of the insurance accordingly.
